ENGINEERED SOLUTIONS OF GEORGIA PROPOSES TO PROVIDE THE FOLLOWING PROFESSIONAL ENGINEERING SERVICES:

Inspection of structural concern as follows:

Concern: Homeowner recently had a concrete slap poured and wants to ensure that a structure can be built on it.

Engineering scope: 1.

Perform one professional engineer inspection of the property to address the structural concern outlined above, collect photos of the structural item of concern, and take level measurements if required. Includes 1 – 1.5 hrs of the Engineer’s on-site time. If the problem is more complex requiring additional time an additional fee will apply. The additional fee will be discussed, and a new contract will be issued prior to commencing the additional work. 2. Document structural concern in an engineering inspection report with observations and recommendations of a solution. 3. No repair drawing is included but can be added as additional scope of work if required. 4. Engineering does not include waterproofing issues. A separate waterproofing appointment will be scheduled with one of our waterproofing experts following issuance of the professional engineering report.

TERMS AND CONDITIONS FOR PROFESSIONAL ENGINEERING SERVICES:

In consideration of the payment of the professional services fee, Engineered Solutions of Georgia (ESOG) agrees to conduct a structural inspection and prepare a written Inspection Report (Report), if applicable, to determine the significance of and provide general solutions for possible structural deficiencies in the areas specifically designated by the Customer. ESOG performs the inspection and prepares the Report for the sole, confidential and exclusive use and possessions of the Customer. The Customer understands and agrees that the inspection will be of the readily accessible areas of the property, is limited to visual observations of apparent structural conditions existing at the time of the inspection only and is based on the professional opinion of the inspector. No finishes are removed and no material testing is performed as part of this investigation. Excluded from the inspection are latent and concealed defects and deficiencies. The structural inspection will be completed at the property location designated on the Jobsite Information section of this contract. All inspection information will be included in the Report. ESOG accepts no responsibility for use or misrepresentation by third parties of the inspection of the Report. Other items pertaining to the property may be discussed, but they are not part of the structural inspection or Report. The inspection is not a compliance inspection or certification of compliance with past or present governmental codes or regulations of any kind. The inspection and Report do not address and are not intended to address the possible presence of, or danger from, any potential harmful substances and environmental hazards including, but not limited to: mold in air and carpet, radon gas, lead in paint, lead in water, asbestos, urea formaldehyde and toxic or flammable chemicals. The inspection and Report do not cover or address swimming pools, wells, septic systems or the presence of rodents, active termites or other insects. The parties agree that ESOG, its employees and agents assume no liability or responsibility for the cost of repairing or replacing any unreported defects or deficiencies that are either current or arise in the future, or for any property damage, consequential damage or bodily injury of any nature. The inspection and report are not intended to be used as a guarantee or warranty, expressed or implied, regarding the adequacy, performance or condition of any inspected structural item. The Customer understands and agrees to limit the liability of ESOG, its employees, officers, principals and agents arising out of ESOG service, inspection or Report to the amount of the inspection fee paid by the Customer. This Agreement represents the entire agreement between the parties. Changes or modifications to this agreement shall be in writing and signed by the parties. This agreement shall insure to the benefit only of the parties signing this agreement and shall not insure to the benefit of any successor of either party. There is no charge for cancellations made 2 or more business days before the appointment. Cancellation requests made the business day before or same day as the appointment require full payment
